Description
MHC Nursing and care services is recruiting Experienced Care Coordinator within Domiciliary team within the Warwickshire region. This position is Full-Time.
We are seeking someone with similar experience, who has experience in organising care throughout the homecare system, rota scheduling, assessing the needs of service users, developing and implementing of support plans, supervising and support care staff.
Start Date: 1st August 2025.

Role and responsibilities:
- Scheduling of staff rotas, ensuring calls are covered
- Developing and reviewing of support plans and risk assessments
- Assessment of new referrals
- Act as mentor/supervisors for junior staff
- Support with training, induction, onboarding of new starters
- Support the recruitment, training, and supervision of care staff
- Support with compliance for health and safety regulations
- Liaise with service users, families, and healthcare professionals
- Provide support with on-call service when required
You are required to have the following:
- Minimum Level 3 in Health & Social Care or willing to work towards this
- Relevant experience as a Care Coordinator or Care Supervisor.
- Strong rota planning and organisation skills
- Driving Licence & Access to own Vehicle
