SUPPORT PLANNER – CONNECT, BASSETLAW, PERMANENT, 14HPW, GRADE 3 CORE – £25,235 FTE
- We are a small, welcoming, and friendly team.
- Flexible working within our service delivery times, 9 AM 5 PM, Monday to Friday.
- The team has a base in Worksop but there is the opportunity for hybrid working.
- Access to a wide range of training.
Connect is a community-based service offering support to people living within the Bassetlaw area of Nottinghamshire. We primarily work with older people and neurodiverse adults.
Our support is delivered in the community, mainly in peoples homes and at local hubs. We also support individuals leaving hospital, to return to their homes safely and maintain as much independence as possible. Support is often delivered in partnership with other agencies, making effective communication essential.
Ideally you should have experience or a good knowledge of the support needs of vulnerable adults living independently in their homes, be able to identify barriers, and have a good understanding of person-centred working.
We are looking for individuals who are:

- Willing to be adaptable and flexible to meet a persons support needs.
- Able to work independently and show initiative.
- Self-motivated and have a proactive approach to problem solving.
- Willing to support team members as part of a small team.
The role would require you to:
- Have the knowledge or be willing to learn how the benefit system works.
- Be confident in completing forms online and on paper.
- Use Microsoft Office – Outlook, Word, Excel.
- Keep accurate records.
- Use the internet for resources and fact finding.
- Attend in-house and external training to keep up to date and relevant.
