I’m delighted to be working with a small, high-quality domiciliary care provider in Hexham who are now looking to welcome a Care Coordinator to their friendly team. With over 40 years of combined leadership experience, the directors offer a very supportive environment.

Care Coordinator Responsibilities:

Managing and scheduling staff rotas to ensure consistent and effective care delivery across the service.
Responding to last-minute care changes or emergencies to minimise disruption to service users.
Communicating regularly and professionally with clients and their families to ensure their care needs are being met and any concerns are promptly addressed.
Liaising with pharmacies to ensure timely and accurate delivery of medication for service users.
Supporting the Registered Manager with the completion of client assessments and risk assessments to maintain safe and person-centred care planning.
Building strong relationships with care staff to promote high standards of care.
Ensuring all documentation and records are kept up to date and in line with CQC regulations and internal policies.
Care Coordinator Requirements:

Experience working in a Care Coordinator role within domiciliary care for a minimum of 12 months.
UK Driving Licence and use of own car.
