About the Service
The “It Takes a Village” (ITAV) program is a Southwark Council initiative aimed at reducing health and social inequalities by improving access to inclusive, coordinated care for those with complex needs. ITAV partners with a network of statutory and voluntary sector organisations to create innovative solutions that support every individual’s unique journey. Our mission is to ensure that people facing multiple disadvantages can access timely, compassionate care that addresses their needs holistically, breaking down barriers to fair and equitable treatment.
About the Role
This opportunity places you at the forefront of meaningful change, empowering individuals and collaborating with professionals to enhance support for those facing multiple disadvantages. As an ITAV Multiple Disadvantage Coach, you’ll lead efforts to deliver integrated, compassionate care and facilitate service improvements across Southwark.
Key responsibilities include:
- Coaching and collaboration – Partner with agencies, statutory services, and community groups to build cohesive networks that work effectively around individuals’ needs.
- Conducting holistic assessments – Gather a full picture of each person’s needs to support entry into preventative, person-centred services.
- Advocacy and pathway facilitation – Ensure individuals receive the respectful, coordinated support they deserve by identifying service blockages and providing practical solutions.
- Service improvement – Contribute to feedback processes and create recommendations for system-wide changes to promote equality and inclusivity in health and social care.
About You
We are looking for a driven and experienced professional who shares our commitment to social equality. You should bring:
- A background in fields such as social work, mental health, or therapeutic practice.
- Strong analytical skills and experience in conducting assessments and designing improvement strategies.
- A collaborative spirit and the ability to build relationships across multiple sectors.
- A compassionate approach to engaging with individuals and teams, with an understanding of trauma-informed, person-centred care.
