Overview
The Case Worker plays a vital role in providing support and guidance to individuals and families in need. This position involves assessing client needs, developing tailored intervention plans, and ensuring that clients receive the necessary resources and services. The Case Worker will also be responsible for supervising and mentoring junior staff, fostering a collaborative environment to enhance service delivery.
Duties
- Conduct comprehensive assessments of clients to identify their needs and challenges.
- Develop, implement, and monitor individualised care plans in collaboration with clients and relevant stakeholders.
- Provide ongoing support and advocacy for clients, ensuring they have access to appropriate services.
- Maintain accurate documentation of client interactions, progress, and outcomes in accordance with organisational policies.
- Supervise and mentor junior staff members, offering guidance and support to enhance their professional development.
- Collaborate with community resources, agencies, and organisations to facilitate client access to services.
- Participate in case conferences and multidisciplinary team meetings to discuss client progress and strategies for improvement.
- Stay informed about relevant legislation, policies, and best practices in social work and case management.
Qualifications
- Experience working within Remortgage
- Previous experience in case management or a similar role is highly desirable.
- Strong interpersonal skills with the ability to build rapport with clients from diverse backgrounds.
- Excellent organisational skills with attention to detail in documentation and reporting.
- Ability to work independently as well as part of a team, demonstrating strong leadership qualities when supervising others.
- Knowledge of community resources and services available for clients is an advantage.
- A commitment to continuous professional development through training and mentoring opportunities.
