ROUGH SLEEPER TEAM RECOVERY WORKER, LINCOLN, PERMANENT, 37HPW, £25,116 – £29,383, FTE

An exciting opportunity has arisen to join Lincolnshire Recovery Partnership Substance Use Services as a Rough Sleeper Recovery Worker. You will work flexibly across Lincolnshire Recovery Partnership supporting individuals who are rough sleeping or at risk of rough sleeping. You will see them whilst providing outreach and visiting housing providers and local charities, delivering brief interventions and engaging them in treatment.
Day-to-day you will work alongside other Recovery Workers, Rough Sleeper Team Manager and Leader. The role also requires you to engage with partner agencies to provide outreach to clients and a multi-agency approach to clients support.

Please Note: A Full UK Driving License and Access to a Vehicle is an Essential Requirement. You will be required to use your own car to travel between sites on a daily basis (expenses will be provided.)
Duties & Responsibilities
- Effective case management of service users with substance misuse issues
- To carry out comprehensive substance misuse assessments including assessment of personal strengths and identification of any risk factors
- Formulate, monitor and review risk assessments and risk management plans that effectively safeguard and protect service users and families/carers from harm
- To develop person centred recovery plans using a strengths-based approach in partnership with service users and others involved in their support network
- Maintain a high standard of case notes and prepare and present narrative reports as required
- Consistent, timely and accurate collection, recording and reporting of data and information relating to a client treatment journey
- Effective multiagency working with statutory and non-statutory agencies in relation to the wider presenting needs and complexities of service users
- Deliver and support harm reduction interventions, including needle exchange and access to physical health care including BBV and health screening
- Ensuring all paperwork and recording in accordance with Turning Point policy and procedure and follow information governance guidelines.
