Permanent, Full Time (37.5 hours per week)

Pay £12.61 per hour (£24,664 per annum) plus great benefits including Health Cash Plan!

Typical day as a Lead Support Worker (known by us as Coordinator)

  • Creating support plans with your customers and ensuring that these are actioned to the highest possible standard.
  • Youll assist people that have recently been discharged from psychiatric hospitals
  • Helping customers with daily living activities such as making meals, shopping, budgeting, maintaining their tenancy and moving onto independent accommodation if they are able.
  • Carrying out risk assessments, support planning, goal setting and regular reviews.
  • Cuppas, chats and catch ups. Theres no rushing from one customer to the next here! Plenty of time to collaborate with colleagues too.

You bring

  • Passion to support our customers to live their best life, working collaboratively with an eye for detail.
  • Experience of creating person-centred support plans and supporting colleagues in working to work to the plans.
  • Experience of coordinating and assessing customer referrals.
  • The ability to work on your own initiative, remaincalm under pressure and have a resilient approach

Job details

  • Where were flexible with you, we do need to meet the needs of our customers. That may mean adapting your start or finishing times on occasion.
  • Rota working Early, Mid & Late Shifts with the possibility of a very occasional night shift. Early Shifts are 07:00 – 15:00, Middle Shifts are 09:00 – 17:00 and Late Shifts are 14:00 – 22:00.
  • Able to use technology for creating and updating support plans, complete online learning and to collaborate with colleagues.
  • Youll need an Enhanced DBS check done and we pay for that.
