The post is subject to a probationary period of 6 months.Background For 20 years, the Swansea Carers Centre has supported a range of carers and grown to be one of the largest carers services in Wales.The Hwb aims to make it easier for carers to access all the services available to them in one place and attract new carers who might be unaware of their rights and entitlements, such as those from ethnic minorities and male carers.The Hwb is a single point of contact for carers in Swansea, offering initial assessments for unpaid carers, and jointly developing a support plan to meet their identified need.The outreach worker will be part of the Carers Hwb team and will work to engage with carers who are unable or don't want to access the city centre base.They will focus on trying to engage carers who are underrepresented such as those from ethnic minorities, male carers and those served by poor public transport links. The Outreach worker post is funded as a part of a National Lottery Community Fund project.
